Tr137 Narrative Report Example - 745 Words

Mike, I dont even know where to begin, but this is completely unacceptable. My truck TR137 has been down since the end of my shift on Thursday, June 23rd, on my post trip inspection I noted an oil leak and steer axle shock had come unbolted on the bottom, both on the passenger side, both of these items I noted on my VCR. I arrived at the yard on Friday, June 24th at my regular time. On my pre-trip inspection, I noticed that the shock was fixed and that the side of the engine with the leak had been washed, but on the VCR it was only noted that the shock was fixed. I questioned the mechanic on duty, Bill, about the oil leaked, he said they could not find it, but go ahead and run it, well see what happens. The hydraulic connections are leaking on the rear of the truck, so much that there was a puddle under it. See pictures below. Other DOT violations included no current fu el tax permit in permit book (although correct stickers are on the truck), tractor ABS light is on the dash, hydraulic hoses are secured to the truck with bungees, passenger door does not close properly. Another issue that prevented the truck from being ready for operation was the locking bar for the fifth wheel plate, that is needed to run our dump trailers was missing. This brings us to the interior of the cab, and this unto itself is unacceptable! This is the amount of garbage I pulled out of the cab. In case you are wondering, that blue thing is a propane canister, that was just rolling around under the garbage along with an unsecured fire extinguisher and yes a 20lb dumbbell. I do not believe I need to tell you that the companys standing with DOT is not great. Saying this truck is ready for safe operation is completely unacceptable. Furthermore, my life, my safety, and my familys financial well-being, as well as every other driver, depends on proper maintenance of the equipment. The cutting of corners and shoddy work, just to get the job done, does a disservice to every person who operates equipment at this company, those of us who show up and do our jobs properly.
